If there is ever a defect in workmanship or materials (such as porcelain fracturing), we will replace your crown (or veneer or onlay) at no charge, up to five years after placement.

Visit our office for a minimum of 2 (4 if we’ve recommended due to gum condition) times per calendar year for cleanings. Note: Many of our patients still return to us after moving to different areas. If you move out of the South Florida area, or are a seasonal resident (we love our “Snowbirds”!), one cleaning and check-up in our office will be sufficient to maintain this agreement!
Complete (in our office) all recommended treatment in the quadrant (quarter of the mouth the tooth in question is in), all within a one year period. Gum infection and bacterial decay spread from tooth to tooth.
Your path to optimal health requires a comprehensive approach to care. Advise us of the need to change or cancel appointments at least 48 hours in advance. Shorter notice cancellations or charges severely limit our ability to offer another patient their most preferred time. You may request a copy of our Treatment Guarantee at you first visit.
If, within five years of placement, our crowns, porcelain veneers, or onlays need replacement due to defects in workmanship (manufacturing) or materials, and the tooth is still savable…as long as Dr. Glerum maintains her practice in dentistry…and you do your part (above) we will at no charge:
Note 1: If a veneered or onlayed tooth is damaged or decayed to the extent that a crown is necessary, the full fee originally paid for the veneer or onlay will be applied toward the crown. You will pay only the difference. We cannot predict if and/or when gum or nerve treatments may be required. If needed, they would require additional fees.
Note 2: This guarantee applies only to individual crowns, veneers, or onlays. Due to their complex nature, fixed (cemented) bridgework and fixed splints attached to implants and/or partial dentures are not included. We will always treat our patients fairly in the unlikely event that treatment must be re-done.
Note 3: Teeth that have had root canal treatment (endodontics) are excluded from this guarantee. Although most root-canal treated teeth last many years, they are more brittle internally than “live” teeth. There is more chance that these teeth can fracture.
